Exhibition Match Request Form

Game Day Equipment Checklist

Injury Report Form

MLQ Championship Stay and Play Policy
All team affiliates and season staff of MLQ traveling to the MLQ Championship are held the MLQ Stay and Play Policy. This policy updated prior to each season.

MLQ Championship Stay and Play Waiver
All team affiliates and season staff of MLQ traveling to the MLQ Championship are held the MLQ Stay and Play Policy. The primary purpose of this waiver is to allow individual players to stay with close relatives who live in the area. Exemptions will be made on an individual basis; this waiver may not be used to grant exemption to a full team.

MLQ Code of Conduct
At MLQ, players are held to a high standard of personal conduct. It is a privilege to be a member of this league. Because of this, everyone who is part of the league must refrain from “conduct detrimental to the integrity of and public confidence in” MLQ as set out by the MLQ Code of Conduct.

Team Radius Waiver
In order to play for an MLQ team while living outside of its defined radius for the majority of the season, it is necessary to submit this form and gain approval from the league.

The Tiebreaker Policy outlines what takes place in the event of tie between MLQ teams.

Uniform Policy
All players on a team’s 21-person active roster must wear the full team uniform at any time they are in the player area to participate in a game. The Uniform Policy further outline athlete kit requirements.
